Enrolment checklist
We require the following to be completed, returned or sighted.
Sight student's birth certificate
Proof of Address in Catchment (Lease Agreement, Electricity Bill, Rates Notice)
Student enrolment form
Permission forms attached with enrolment paperwork
SRS Agreement form
Bank Details form
Q Parents form
Subject selection form (if required)
Any other relevant paperwork
Medical information
Please ensure we have any important and/or relevant Medical Information including any current medications regarding your student on file in case of emergency.
Any student who requires a health plan for a medical condition must contact the school office as soon as they are made aware of it's requirement. The school will guide you through the process of how health plans are managed at our school.
If your child is required to take medication whilst at school or on school activities such as camps, you need to contact the school office to discuss the correct process for your child's requirements.
The school will not administer any medication to a child unless the correct process has been followed.
