New Paperwork Process - Port Curtis Trials
As sport comes to an end for 2022, preparation has already begun for 2023. Next year, we are trialling a new process around consent forms which will streamline the processes from school to state levels. The new Representative School Sport Permission Details Booklet (Consent Form) has all required documentation that schools/districts/regions require for your child to participate. To minimise the duplication that formerly existed, Toolooa are now asking you to electronically complete the form online once only per sport/trial.

If you are unable to access or print the necessary booklet, please don’t hesitate to contact myself to assist. We encourage you where possible, to fill out the form electronically to assist with the eligibility and accuracy of the documents. In addition to this, all Port Curtis trial details, including the link to the Capricornia School Permissions Detail Booklet can be found on our school website under the ‘Extra-Curricular – Port Curtis Trials’ section.
Please note, that parent/carer, sports coordinator and Principal consent (or delegate) is required for your child to nominate for trials. All pages of the forms must be returned via email to myself, or handed in to the PE staffroom. Students will NOT be representing Toolooa at Port Curtis Trials if paperwork has not been returned by the due dates advertised.
As we have a large number of Port Curtis trials within the first few weeks returning to school in 2023, I encourage you to complete and return paperwork for any trials that your child may be interested in as soon as possible. Please see the Port Curtis Trial overview for Term 1, 2023 below;
